Interprofessional Collaboration for Improving Patient & Population Health Outcomes

This Essential demonstrates work as a member and leader of interprofessional teams; communicates, collaborates, and consults with other health care professionals to manage and coordinate care across systems and environments.

I feel I have met this essential by merely working in the health care field. Being the charge nurse of my facility's Med-Surg unit, I meet daily with the hospitalist, respiratory therapist, case management, employee health nurse, physical therapy, and other members of the health care team. We collaborate and strive for the best outcome for our patients daily. Examples of my educational collaboration are listed below in my assignments list. Assignment 1 is a patient education resource for Alzheimer's that I did in Pathophysiology, and Assignment 2 is my group's HIT project from Healthcare Informatics. My group and I collaborated, researched, and constructed a website to be used as an educational resource on COPD. The website is included below.

Interprofessional Collaboration for Improving Patients & Population Health Outcomes

Additional assignments I used to show how I met this competency include The Health Plan Case Study from Health Policy and Law and my comprehensive SOAP note from PEDs.

I feel I met this essential by doing extensive research. My project partner and I researched healthcare in Germany and the differences in the type of healthcare coverage, access to care, cost, and preventative services compared to the United States.

For my SOAP note, I feel I met this essential by having the opportunity to educate my patient's parents and help improve the outcome of a 2-month-old male where the mother states she did not get the patient circumcised, and was nervous about his ability to urinate.
